#24 The Whole Person Prescription: Lifestyle Medicine with Dr John Sykes

Dr Elizabeth Thompson speaks with GP and lifestyle medicine advocate Dr John Sykes, about why true health goes far beyond managing biomarkers. Drawing on his work both in the NHS and private practice, John makes the case for whole person care, addressing physical activity, nutrition, sleep, mental wellbeing, harmful substances and social connection as the real foundations of health. The conversation explores the growing role of GLP-1 weight loss medications, where John argues passionately that wraparound lifestyle care should be the core treatment, not an afterthought. He touches on the battle between big food and big pharma, the critical importance of muscle mass as we age, and the science of muscle myokines as natural anti-inflammatory agents. The episode closes on an optimistic note, celebrating community-based initiatives like Park Run and the growing momentum behind lifestyle medicine as a genuine force for change.

Key Topics Discussed:

  • True health means physical, mental and social wellbeing working together, not just the absence of disease or controlled markers.
  • Lifestyle medicine focuses on six evidence-based pillars: physical activity, nutrition, sleep, mindfulness, avoiding harmful substances and social connection.
  • Around 70% of chronic diseases are rooted in lifestyle factors, many of which can be significantly reduced, slowed or even reversed.
  • Accountability, whether through a coach, a friend or a community group, is one of the biggest drivers of lasting behavioural change.
  • Loneliness is a significant predictor of early mortality, making community and social connection as important as diet and exercise.
  • Muscle myokines, released during exercise, act as powerful anti-inflammatory agents that support whole-body health at a cellular level.
  • Resistance training from midlife onwards is essential to combat sarcopenia, the gradual age-related loss of muscle mass.
  • Muscle strength, particularly leg strength and grip strength, is one of the strongest predictors of a long and functional life.
  • Physical activity’s greatest value is not weight loss but its dramatic reduction in risk across diabetes, heart disease, cancer, depression and dementia.
  • Stopping GLP-1 medications without proper lifestyle support can leave patients in a worse position than when they started, due to muscle mass loss and rebounding appetite.
  • Wraparound lifestyle care should be the foundation of obesity treatment, with medication acting as a support tool rather than the primary solution.
  • Personalisation is everything; lifestyle advice only works when it fits the individual’s real life, preferences and circumstances.
